Parental Consent Form

I ______________________ being the parent (as described below) of __________________ give consent for

___________________________ to take my child on a Discovery Cruise to Freeport, Grand Bahama Island on

_______________ day of ______, 200__, returning _________________ day of, 200__.

I declare that ___________________ is my child by either natural birth, adoption, or that I am the child's legal guardian and will supply whatever documents that I need to show security personnel at the pier. Required documents would be original or certified copy of birth certificate and if necessary original adoption papers or other legal papers showing legal guardianship. Children 16 and over also need a government issued picture ID such as drivers license or school ID.

I also understand that this Parental Consent Form must be signed by me in the presence of a notary and that only the original of this document will be accepted.
